Requirements:
- Filipino Citizen
- Minimum age of 18 (High School or College Graduate)
- 3 copies of Diploma or Transcript of Records
- 6 copies of Any ID (with 3 signatures)
- 1 copy of Birth Certificate
- 2 copies of Student Permit or Driver's License
- 7 pieces passport size pictures (white background, formal attire, with nametag)
