Requirements:
- Photocopy of the following:
- Any valid ID (with Las Pinas City address) such as UMID Card (GSIS/SSS), T.I.N., Driver's License, PRC, Voter's ID, Postal ID, and Green Card
- Birth Certificate
- Residence Certificate (CEDULA)
- Two (2) pieces of 1x1 colored picture
- TESDA COC (Practicing COVID-19 Preventive Measures in the Workplace)
- Gmail Account (Preferably)
