- If you're a married female applicant, include a photocopy of your Certificate of Marriage
- Provide a photocopy of your Report Card, Transcript of Record, or Certification from the last school you attended
- Obtain a Barangay Clearance
- Get a Medical Certification confirming your Fitness for Training
- If applicable, submit a photocopy of your National Career Assessment Examination results
