Qualifications:
- Must be 18 years old and above
- High School Graduate (K to 12)
- ALS Completer
- College Graduate
- College Undergraduate
Requirements:
- 2 pieces of 1x1 picture with collar and white background
- 2 copies of Photocopy of PSA birth certificate (For married individuals: Photocopy of PSA marriage contract)
- 2 copies of Photocopy of valid I.D
- 2 copies of Photocopy of TOR-Certified true copy (College graduate/College Level)
- 2 copies of Photocopy of Diploma and Form 137/138 (High School Graduate)
- 2 copies of Photocopy of Grades with certificate of completion (ALS Completer)
