Training Requirements:
- Age requirement: 18 to 55 years old
- Educational background:
- High School Graduate (Submit a photocopy of SF9 or Form 137 & High School Diploma)
- ALS Graduate (Provide a copy of Certificate)
- College Graduate (Include a photocopy of Transcript of Records (ToR) and College Diploma)
- Supporting documents:
- Photocopy of PSA Birth Certificate
- Photocopy of 1 valid government ID with 3 Specimen Signatures
- 4 pieces of 1x1 ID pictures
- 6 pieces of passport-size ID pictures (strictly with a white background, in a white collar with a name tag)
- Long brown envelope
