Requirements:

- 3 pieces of passport-size ID photos with a nametag, white background, and formal attire
- 3 photocopies of Birth Certificate or Marriage Certificate (if married)
- 3 photocopies of TOR/Form 137/Form 138
- 6 photocopies of 1 Valid ID with 3 original signatures (per copy)
- 1 copy of Updated Resume
- 2 copies of Practicing COVID-19 Preventive Measures in the Workplace (visit: https://e-tesda.gov.ph/)
