Requirements:
- 2 pieces of 1x1 ID photos with a white background
- 2 passport-size IDs with a nametag, white background, and formal attire
- 3 photocopies of your Birth Certificate or Marriage Certificate (if married)
- 3 photocopies of TOR/Form 137/Form 138
- 6 photocopies of 1 valid ID with 3 original signatures (per copy)
- 1 copy of your updated resume
- 2 copies of Practicing COVID-19 Preventive Measures in the Workplace (visit here)

- At least 18 years old
- Completed at least 10 years of basic education or holds an ALS certificate of achievement equivalent to grade 10
- Filipino Citizen
