- Barangay Clearance, Police Clearance, or NBI Clearance to establish your good standing in the community.
- A Medical Certificate issued by a Public Health Officer ensuring your fitness for the program.
- Photocopy of your Birth Certificate (PSA) if married.
- Passport-sized pictures (4pcs.) and 1x1 pictures (6pcs.) with a white background, featuring a collar, nametag, and chemically printed for a polished appearance.
