To be eligible for the scholarship, you need to provide two (2) photocopies of the following documents:
- Form 137/TOR/Diploma,
- Birth Certificate,
- Barangay clearance,
- and Pictures in white background, with collar attire (identical): four pcs. 1x1, four pcs. 2x2, and five pcs. Passport size.
All of these must be fastened in a long RED folder. Don't forget to present the original documents upon enrollment for verification.
