Requirements:
Photocopy of the following:
- Cedula
- Voter's ID/ Voter's Stub/ Voter's Certification/ Real Property Tax
- Birth Certificate/ Baptismal
- Recommendation from the Barangay Chairman
- Copy of grades of the last school attended/ Diploma
- Covid 19 Vaccination Card.
