• Photocopy of Diploma
• PHOTOCOPY OF PSA BIRTH CERTIFICATE
• 4 PHOTOCOPIES OF ONE (1) VALID GOV'T./SCHOOL I.D.
• (for Female) PHOTOCOPY OF MARRIAGE CERTIFICATE
• PHOTOCOPY OF VACCINATION CARD
• 4 PCS. 1x1 I.D. PICTURES
• 6 PCS. PASSPORT SIZE I.D. PICTURES

(NOTE: required strictly with white background, with collar, with name tag (full name including middle initial) and in matt finish) 

Submit the requirements personally @ Asian College 1013 Aurora Blvd. Project 3 Quezon City. Mon-Fri 8am-4pm.
