* Transcript of Recors (TOR) for College Level or Graduate (3 copies)
* NSO or PSA or birth certificate (2 copies)
* any valid I.D (5 copies)
* 1 pc long brown envelope & plastic envelope
* 8 pcs passport size picture w/name tag, white back ground, corporate attire, type of photopaper glossy front and back)
