Requirements for enrollment:
1. PSA/NSO Live Birth (photocopy)
2. Diploma/T.O.R./Form 137 (photocopy)
3. Medical Clearance (Physical Examination only)
4. Police Clearance/NBI Clearance (photocopy)
5. (3) three pcs. Passport size I.D picture
6. (3) three pcs. 1x1 I.D picture
7. Photocopy of NC II (for TM I enrolees)
8. Aptitude Test
